HIGH SCHOOL VISITATION REQUEST FORM
Each eighth grade student is allowed and encouraged to visit one or two area Catholic high schools as
they make their decision about school for next year. They may be excused from school for one or two
days to experience a day at one or two of these schools. Students who wish to visit a third school are
encouraged to find a day that St. Clare is not in session but the high school in question is in session.
Parents and students together should complete this form requesting dates for high school visitation.
Dates must be checked at the high school first and then approved by the St. Clare teachers. A parent
needs to sign the form before teachers will check the dates requested. 8th grade students need to check
the date(s) available with Mrs. Kollars first, and they let their other teachers know that they will miss
class on the approved day(s). Typically no more than 3 students will be allowed to be absent to visit
high schools on the same day. Of course, student will be responsible for making up all school work
on the days they are visiting high schools.

Return completed form to Mrs. Kollars. While signatures are not necessary from other teachers, (Mrs. Cree,
Mrs. Acurso, Mrs. Sherbo) students are still responsible for making up any work missed in those classes
as well. Students should also be aware of when an absence in any class could cause complications in a group
or individual assignment.
